Does UCA own urgent care centers?
Though UCA fully expects to advance to the ownership level with physician partners that see fit to involve us as a preferred management solution, it is currently pursuing its mission to be a synergizing force for physicians in operating their centers that they or their partners own. UCA brings together the best available resources in the marketplace to deliver virtually every possible service to make the owners of urgent care centers the most successful. UCA management recognizes that individual operators do not necessarily have the time to spend on bringing these resources to the degree that it can and therefore it fills a void no other company does.

Should Urgent Care physicians be able remove foreign bodies from the eye?
Urgent care physicians should be able to remove foreign bodies from the eye. The physician should attempt to remove the foreign body; if unsuccessful then he/she should refer the patient to an ophthalmologist or the ED. Foreign bodies penetrating into the eye (deeply embedded) is beyond the scope of the Urgent Care physician and need to be referred to an ophthalmologist or the ED. Rust rings need to be taken care of by an ophthalmologist.

What options are available for physicians who want to relocate to Health Management hospital communities?
There are basically two distinct models that Health Management hospitals use to relocate physicians to their communities; recruitment to private practice is, by far, the more popular and widely used of method of recruitment. Under this model, the local Health Management hospital may provide a recruited physician with financial assistance, for a limited period of time, to help the recruited physician establish and maintain a private medical practice. One common form of financial assistance is a "cash Collections guarantee", whereby each month, usually for about 12 months, the Health Management hospital pays the recruited physician the difference between the monthly guarantee amount (an amount agreed to in advance by the hospital and recruited physician, based upon many factors such as the community’s need for the recruited physician’s specialty, the recruited physician’s experience, and the average earnings for similarly situated physicians according to independent surveys) and the amount of revenue that the recruited physician actually collected during that month. Provided that the recruited physician maintains his or her private practice in the recruiting hospital’s community for at least three years, recruited physicians are typically not required to repay the hospital for any of the financial recruitment benefits, other than amounts that are specifically designated as loans and that are expressly subject to repayment. Other types of financial assistance may include commitment bonuses, practice-start up expenses payments, and other amounts.
Employment is the alternative model. Employed physicians are usually paid a base salary for one year, and thereafter, are compensated based on their individual productivity.

How do Health Management hospitals determine whether there is a need for physcians of particular specialties in their respective communities?
All Health Management hospitals engage independent companies specializing in physician needs assessments in order to recommended physician levels for each relevant specialty based on community demographic information, utilization, attrition analysis and standardized physician-to-population ratios. Before any Health Management hospital agrees to recruit a physician of any specialty to its community, there has already been independent expert verification that the community has a need for a physician who practices that specialty. In Health Management’s experience, physicians who relocate to and establish their practices in communities that have demonstrated demands for additional physicians increase the likelihood that their medical practices will be successful.

What is the recommended patient-per-hour ratio for Urgent Care physicians?
The average patient-per-hour ratio for Urgent Care physicians is 4.5 patients per hour. This will vary on the severity or acuity of the patient's conditions, however it is a reasonable goal. Criteria are difficult to assess: if a center implements Electronic Medical Records (EMR) with templates and has protocols in place, the number can go up to approximately six to eight patients per hour.

What are the costs for affiliation with UCA?
UCA derives its fees based upon services provided to affiliated urgent care clients. The fees charged change from time to time but are generally based upon the following categories:
1. Consulting Services: Examples of these fees include: Site selection, business plan preparation, assistance in securing financing, operations analysis, problem solving, compensation designs, project development, valuations, etc.
2. Credentialing/Managed Care contracting: This includes credentialing for all payors for both start ups and on-going enterprises.
3. Billing/Collection/Coding: We have central billing operations in Prairieville, LA and Cincinnati, OH. We have a host of models that provide for turn-key products to include EMR/PM/Hosting or just Billing/Collections services.

What Physician Quality Reporting Initiatives (PQRI) are applicable to Urgent Care?
Physician Quality Reporting Initiative is a pay-for-reporting program that uses measures to gauge the level of care provided by physicians to Medicare patients. Physicians who meet or exceed the minimum reporting standards may be eligible for a 1.5% bonus (subject to cap). There are 119 PQRI quality measures as determined by the Centers for Medicaid Services (CMS). The following excerpt from the CMS website outlines the measure reporting process:
If there are not more than three quality measures applicable to the services provided by the eligible professional, then each measure must be reported for at least 80% of the cases in which the measure was reportable.
If there are four or more quality measures applicable to the services provided by the eligible professional, then at least three measures, selected by the eligible professional, must be reported for at least 80% of the cases in which each measure was reportable.

Can physicians whom Health Management hospitals recruit join established medical practices, or must they start solo medical practices?
Health Management hospitals want their recruited physicians to establish a medical practice in a manner that will best suit their own needs and the needs of the community. Health Management hospitals generally regard established medical practices as ideal practice opportunities for new recruits, and will support recruited physicians’ efforts to find opportunities with established medical practices.

What is the general theme of Health Management relative to medical staff development and to what services is Health Management most committed?
The medical services that each Health Management hospitals offer may vary. Generally, however, each Health Management hospital offers at least the following services: emergency departments, general surgical services, medical services and outpatient services (imaging, lab, etc.). Some, but not all, of the Health Management hospitals also offer more specialized services, including obstetrics, open heart surgery, radiation oncology, neurosurgery, bariatric surgery, pediatrics, urology, rehabilitation services, and other services.
